Cambridge Community Outreach Group (CCOG) is a consultation and assessment service offered to persons with disabilities and/or their families and caregivers in the Western Region of Nova Scotia.The team is made up of clinicians and support staff from Kings Regional Rehabilitation Centre with specific skills, knowledge and experience specifically selected to respond to each accepted CCOG referral. Referrals are submitted by caregivers, recipients of care and supporting agencies.
Consultation or education is designed to enhance independence of the referred individual and to build capacity of families and caregivers to strengthen supportive strategies. Consultations are done both in-person or via telephone.
Services can include but are not limited to:
- Consultation on strategies and interventions for responsive behaviours;
- Consultation on complex needs with assessment and recommendation on further needs;
- Professional development opportunities on a fee-for-service basis relating to topics such as Mental Health First Aid or Behavioral Support; Assistive Technology and Communication; and a variety of mental health disorders.
The program is delivered in collaboration with the Services for Persons with Disabilities Program (SPD) of the Nova Scotia Department of Community Services.

Completed referrals are to be faxed (902-538-7022) or mailed. Note that client specific referrals also require completion of the Authorization for Cambridge Community Outreach Group Consultation and Release of Information.
