Cambridge Community Outreach Group (CCOG) is a consultation and assessment service offered to persons with disabilities and/or their families and caregivers in the Western Region of Nova Scotia. The CCOG team is comprised of clinicians and support staff from Kings Regional Rehabilitation Centre. These committed professionals have specialized knowledge and experience. Referrals are submitted by caregivers, recipients of care, and supporting agencies, and staff are assigned based on their specific expertise.
Consultation and/or education is designed to enhance the independence of the referred individuals and to build capacity of families and caregivers to strengthen supportive strategies. Consultations are done both in-person and via telephone.
Services include, but are not limited to: (See printable Brochure at bottom of page)
- Consultation on strategies and interventions to address behaviours of concern;
- Consultation on complex needs with assessment and recommendation re: further supports;
- Professional development opportunities on a fee-for-service basis relating to topics such as Mental Health First Aid, Positive Behavior Support, Assistive Technology and Communication, various mental health disorders.
The program is delivered in collaboration with the Disability Support Program (DSP), Nova Scotia Department of Community Services.

Please note that client specific referrals also require completion of the Authorization for Cambridge Community Outreach Group Consultation and Release of Information.
